Healthcare Planning and Coordination.
Registered Nurse on staff to provide case management services/supports to our Individuals served.
Provide screenings, assessments and evaluations to each individual receiving services.
Transportation to all medical appointments and community activities
Provide a 24-Hour Structured Environment with Substantial Supervision and Assistance/ Care with daily living skills in varies areas.
